Endometriosis
Endometriosis is a condition where tissue similar to the uterus lining grows outside it, often around reproductive organs like the ovaries and fallopian tubes, but can also affect areas like the bowels and bladder.
Symptoms can vary greatly; about 25% of people have no symptoms, while others may have severe problems. Common symptoms include pelvic pain, heavy and painful periods, pain during bowel movements, urination, sexual intercourse, and issues with fertility.
Nearly half of those with endometriosis experience chronic pelvic pain, and it can also affect mental health and social life.

In this study highlighting Pycnogenol, 58 women were surgically diagnosed with endometriosis and were then started on Pycnogenol within 6 months of the surgery after confirming regular menstruation and ovulation for three months. Women were randomized to receive either 1) pycnogenol 30 mg twice daily for 48 weeks or a 2) gonadotropin-releasing hormone agonist (Gn-RHa), leuprorelin acetate depot, 3.75 mg IM six times every four weeks for 24 weeks.
After four weeks on Pycnogenol, patients slowly but steadily improved reducing symptoms from severe to moderate. Overall, this group experienced a 33% reduction in symptoms of their endometriosis.
The Pycnogenol group maintained regular menses and normal estrogen levels during treatment and as expected, the leuprorelin group had suppressed menstruation and drastically lowered estrogen levels during treatment. In addition, five women in the trial taking Pycnogenol became pregnant.

This systematic review and meta-analysis aim to estimate the possible advantage of an extended treatment in the relief of chronic pain. The literature search was conducted consulting scientific databases, to identify clinical trials in which micron-size PEA was administered for at least 60 days, and pain assessed by the Visual Analogue Scale (VAS) or Numeric Rating Scale (NRS). Nine studies matched the required criteria, for a total of 742 patients involved. The meta-analysis showed a statistically and clinically significant pain intensity reduction after 60 days of micron-size PEA supplementation, compared to 30 days (1.36 points, p < 0.01). The secondary analysis revealed a weighted NRS/VAS score decrease of 2.08 points within the first month of treatment. These two obtained scores corresponded to a 35.1% pain intensity reduction within the first month, followed by a further 35.4% during the second month. Overall, these results confirm the clinically relevant and time-depended pain-relieving effect of micron-size PEA and therefore the advantage of an extended treatment, especially in patient with incomplete pain management.

In this non human study treament with CoQ10 treatment significantly decreased the volume of endometriotic implants, VEGF, and MMP-9 immunoreactivity and increased TUNEL-positive cells. The findings of the study suggest that CoQ10 can be used in endometriosis treatment by suppressing the endometriotic implants and reducing endometriosis tissue growth.
